Documents Required: 
  • Written explanation of the damages, signed by the member
  • Pictures of the damage and repairs as they are completed, in color
  • Invoices/estimates/proposals- paid or unpaid (receipts and cancelled checks for proof of payment are also required)
  • Contractor's license
  • Building permit (if required by the city/town) or contractor's explanation of why the permit is not needed
